Service record worksheet

Prepare service record worksheet using your own facts, evidence and verified legal grounds.

What you will prepare

  • Service details: recipient, method, date and place
  • Evidence and exhibit references

Information to have ready

Case reference, court and jurisdiction, party names, author contact details, relevant dates and your supporting exhibits. Provide verified legal grounds where requested; the tool does not invent authorities or facts.

Review before filing

Confirm that this document is appropriate for the proceeding, use any mandatory official form, check confidentiality and redactions, and complete required signatures, swearing and service. A draft export is not a filing or proof of service.

Québec small claims: record what was actually delivered

This is a personal preparation worksheet, not a bailiff’s report or an official proof-of-service form. Exporting sends and files nothing. The checklist below concerns Québec small claims only and does not choose a valid delivery method or calculate a deadline for you.

  1. Identify the exact document and purpose. For the initial Québec small-claims application, the courthouse sends the application to the defendant after the court record is opened. A demand letter is a separate step; do not confuse its delivery with notification of the court application.

  2. In your notes, record the sender, intended recipient, actual method, date, time and address used. Distinguish an attempted delivery, a sent item, a delivery reported by someone else and confirmed receipt. Name the source of each detail; keep unknown information and interpretations separate from observations.

  3. Keep the actual supporting records and reference them accurately. For a demand letter, the official guidance recommends keeping a copy of the letter and receipt evidence, such as the postal delivery confirmation or a copy of the bailiff’s report. This worksheet does not replace those records or establish that notification was legally valid.

  4. Keep failed attempts, returned mail and unresolved receipt questions visible. For an initial application that cannot reach the defendant, follow up with the clerk: the official guidance describes address corrections, public notices and bailiff delivery handled through the clerk. This worksheet does not authorize an alternative notification method. Check the applicable procedure for any other document or jurisdiction.

Fictional example: reported delivery

Entirely fictional; opening this example never adds a fact to your case.

“On 12 March 2026, the tracking record displayed ‘delivered’. I kept a copy of that record. I did not personally see who received the envelope; the recipient’s identity remains to be confirmed.”

A tracking status and your interpretation of it are different. This example does not conclude that service was valid.
