Application for verification of a will — application, sworn statements and notice of presentation
Prepare, from a single entry, the originating application in verification of a holograph will or a will made before witnesses, the applicant’s sworn statement, the notice of presentation and the witness statement.
What you will prepare

- Deceased: occupation, last domicile, date and place of death; proof of death filed as P-1

- Notice of presentation: recipients (names and addresses), exhibits in the same order as the application, place, date and time of presentation

Category and limits
Complete template to fill in yourself · category A · practice area : Estates · offered in case files of type : Estates
Complete template that you fill in for your own file. The fixed text only carries the mentions the cited provisions require; the facts, choices and requests are yours. The tool does not file, serve, swear or send anything, does not choose a remedy or a deadline for you, and guarantees no outcome (Act respecting the Barreau du Québec, s. 128: “to prepare and draw up a notice, motion, proceeding or other similar document intended for use in a case before the courts”, “performed for others”, is the exclusive prerogative of the practising advocate or solicitor).
What the provisions read require

- C.C.P., arts. 306 and 308 — French text: the notice makes mention “du lieu, de la date et de l’heure de sa présentation” and gives “l’indication des pièces”; presentation “ne peut être fixée à moins de 10 jours ni à plus de deux mois après la notification”. (LégisQuébec — Code de procédure civile, C-25.01, read on 2026-10-21)
- C.C.P., art.
- C.C.P., art. 107 — French text: acts to be presented are filed “au moins deux jours avant la date prévue pour leur présentation”; an originating application not notified within three months of filing “est périmée”. (LégisQuébec — Code de procédure civile, C-25.01, read on 2026-10-21)
- Brochure “Demande de vérification d’un testament” (February 2022) — Contents of the file, original and certified copy marked “Certifiée conforme”, filing of all originals at least two days before presentation, endorsements printed landscape. Complete template (category A). The application, statements and notice follow the structure of the Ministry brochure (February 2022, re-read on 2026-10-21) without reproducing its models; art. 772 C.C.Q. and arts. 105, 107, 306 and 308 C.C.P. are quoted as read. Preparation only: the oath, notification, filing and decision remain outside the tool. Québec statutes and most Québec sources linked here were read in French; English wording in this guide is an unofficial summary unless an official English text is linked.
Preuve en main is a preparation and legal-information tool. It gives no legal advice, replaces neither a lawyer nor the official form, and guarantees no outcome.
