The official PDF is neither reproduced nor pre-filled by the tool: Québec government content may not be reproduced without prior authorization (quebec.ca “Droit d’auteur”, updated 10 February 2025, read on 2026-10-21). You copy the prepared answers onto the official form yourself, check each line and sign it.

Time limits read
- Notification at least 10 days but not more than 60 days before the hearing (YPA, s. 76).
Exhibits and documents
- Decision or order to be reviewed.
- Copy of any agreement, application or judgment concerning the child (art. 128).
- List of exhibits with the party letter (art. 125); proofs of notification.
To verify (not read or not settled)
- The SJ-174 (2021-11) still names the “directeur général”; the Act now says “président-directeur général ou directeur général” (2023, c. 34) — 13-01, deduced.
- YPA, s. 74.2 e): no box of the SJ-174 (2021-11) refers to it; to check with the office of the court.
- Use of the SJ-174 before the youth division of the Unified Family Court: NOT READ.
- Court costs: not read.
- Mode of service of the application by a party other than the director: the SJ-174 notes (2021-11) mention registered mail, which s. 76, as amended in 2022 (c. 11), reserves to notification by the director — to confirm with the office of the court or a lawyer.
Help preparing the official form (category B). Box wordings and notes come from the SJ-174 (2021-11) re-read on 2026-10-21; YPA ss. 74.2, 76, 95 and the Court of Québec rules are quoted as read by research 13-01. Québec statutes and most Québec sources linked here were read in French; English wording in this guide is an unofficial summary unless an official English text is linked.
Preuve en main is a preparation and legal-information tool. It gives no legal advice, replaces neither a lawyer nor the official form, and guarantees no outcome.
