Affidavit drafting worksheet
Prepare affidavit drafting worksheet using your own facts, evidence and verified legal grounds.
What you will prepare
- Material facts (one paragraph per fact)
- Evidence and exhibit references
Information to have ready
Case reference, court and jurisdiction, party names, author contact details, relevant dates and your supporting exhibits. Provide verified legal grounds where requested; the tool does not invent authorities or facts.
Review before filing
Confirm that this document is appropriate for the proceeding, use any mandatory official form, check confidentiality and redactions, and complete required signatures, swearing and service. A draft export is not a filing or proof of service.
Québec affidavit: from worksheet to the official form
This is a preparation worksheet, not a sworn affidavit or jurat. The export creates no oath, signature, commissioner identity, seal, court-file number, filing, service or deadline. Québec sources are reviewed for the Québec civil-proceedings context only; other jurisdictions require separate review.
Confirm the proceeding and jurisdiction. Québec’s SJ-1139/SJ-1139A pair is a prescribed civil-proceedings form family; another court, tribunal or proceeding may require a different form.
Write one clear factual paragraph at a time. Mark what you personally observed, what someone else told you and what remains an interpretation or question. Do not fill gaps from assumptions.
List real supporting records and exhibit references without attaching, authenticating or marking them. Keep the original records separate and preserve their provenance.
Review the current official form and the applicable procedural rule. The Québec Code of Civil Procedure limits sworn statements to clear, relevant facts that the person can attest to; the app does not decide relevance or admissibility.
Arrange the oath or affirmation with an authorized person and leave the signature, jurat and commissioner details to that process. Confirm filing, service and any deadline separately with the proper office.
